Cyril's job involves no firearms, poisons, explosives, or powerful enemies. But it endangers his wife. How come?? |

His wife is in danger of dying? Becoming sick? Or being hurt? Kidnapped? Is this grave danger? Or the kind of, "You're in danger of being struck by lightning while you're asleep!" not really dangerous danger? Is she always in danger? Or only when he's at work? Only when he's at home? In danger from another person? From he, himself? From her, herself? From something natural? If he had a different employer, but the same job, would she be safe? If he had different hours, but the same job and employer, would she be safe? If he were payed more or less, would she be safe? If he performed the same job somewhere else, would she be safe? Is he self employed? Is his job in the service industry? Is it a proffessional job? That requires education? Apprenticeship? Is it a high paying job? Is it a job that most people aspire to having? Does he work in an office? In the same building every day? Week? Month? In a different building every day? Week? Month? Outside? Does he know that this job is putting his wife in danger? Did he know when he took the job? If he had known, would he still have taken the job? |

Does this take place in any particular city/village/town? In North America? South America? Europe? Asia? Africa? Australia? Antarctica? Is he the only such doctor withing a long distance, and he is incapable of operating on one he loves as much as his wife, so if she fell sick it would be hard to heal her? Do the couple have any children? Relevant? If they did have children, would the child(ren) be in the same danger as the wife? Is the man a medical doctor? A dentist? Does he specialize in one part of the body? Located in the legs? Arms? Torso? Head? Neck? |

Is he testing these phobias, eg by sending her into a room full of live bats/rats etc? |

You said that besides, Cyril, wife and colleagues there is another relevant person. Is it family? friends of any of the above? Bosses? Authorities? (police, government etc. ) any other people at the workplaces of the two? Business partner? You said that the wife is blackmailed somehow. They are threatning her with her own life? her job? something related to her job? social status? any of her material possesions? her health? her physical or mental integrity? Has she something else to lose if she doesn't cooperate? Material? Spiritual? Abstract? YES!!!! ***SPOILER ********** A particularly outrageous aspect of psychiatry in America (& plenty of other countries too, I bet) is that someone can be committed against his will to a mental hospital simply on the say-so of 2 psychiatrists. So all the husband needs is 2 cooperative colleagues, who may either know they have bad nmotives or just be ready to define unconventionality (or whatever) as mental illness/. Since there are virtually no objective criteria of mental illness, this would br hard for her to fight--unless she's a psychiatrist herself, or a lawyer or judge. In the UK it has to be two doctors and a social worker. One doctor may be (and often is) the patient's own general practitioner and the other must be a psychiatrist. Following this the patient has the right of appeal and will be heard by a "mental health review tribunal" (an independent body) that tends to lean towards the patient. They will invariably grant discharge unless it can be demonstrated that it is not possible to manage any risk of suicide or danger to others, in the community. This is regardless of whether they are mentally ill or not. In fact compulsory detention can only be achieved in the first place if the 2 doctors and the social worker believe that the risk is too great if the patient is left in the community. Patients tend to appeal after they have had some treatment and are partially improved. That means that the risk is reduced and discharge is more likely. Unfortunately it is usually these patients that stop their treatment as soon as they are discharged. They subsequently relapse. If this pattern is repeated they often sustain permenant cognitive impairment and become chronically mentally ill. Having said all of that, any officer of the law has the power to arrest and convey someone to a mental hospital for detention if they believe that the individual is mentally ill and a danger to the public or of self-harm. That detention can last for 72 hours, during which, the two doctors and social worker have to do their stuff if the period of detention is to be extended. Extended periods of detention are: 28 days for assessment (and treatment) 6 months for treatment 1 year 2 years indefinite ("at her majesty's pleasure") |

I agree with the standard objections to this policy, as follows. Detaining someone because he's a danger to the public is preventive detention. A free society doesn't normally allow this to be done to the mentally healthy. So why to the mentally ill? As for self-harm, mountaineers, smokers, & stuntmen are all in danger of self-harm, but we don't intervene in their lives. As puzzle regulars probably know by now, I used to be 2 1/2 times my present (normal) weight. This was probably as risky medically as it would be for a mental patient to go off psychotropic medications, but no one presumed to force me to lose weight. I overate because I loved it, just as I gather mental patients stop taking their meds because they hate the side effects. From what I've read of such side effects, this sounds pretty reasonable to me. The problem is esp. acute in America, where many people classified as mentally ill (I don't know much abt. the UK), know what they're doing & choose to take certain risks. As for suicide, as a future old person, I especially oppose the tendency in America to treat the desire to die as rational in the old & ill (& unproductive & burdensome) but as pathological in the young & healthy. Even if you're more sympathetic to the concept of mental illness than I am,I think it would be hard to deny that it's overused in America. |
