Does this refer to the Mark 14 torpedo again? Other torpedos? Other submarines? Other wartime events? Is there a mechanical problem? Did it cause unintended death? Injury? Does it cause problems with the submarine launching it? |

Did the torpedo explode in the launcher? Just after launch? Was the problem with the explosive? The trigger mechanism? |

Yes! ***SPOILER*** The fourth problem with the Mark 14 torpedo was that it sometimes failed to straighten its course, and instead continued in a big circle until it came back at the ship that fired it. At least one sub was sunk by this malfunction, which was obviously worse when all the other problems had been fixed. |
