Does he work in a hospital? no a practice? yes in research? Actually, this is true as well. But irrelevant to the puzzle.

Yes Was the patient the one who didn't believe it until the other man confirmed it? No one, including the patient, believed the doctor's claim (except the man who confirmed it).

Are there more people relevant to the story than the doctor, the other man and the patient? There are other people in the story, but they are not particularly relevant to this puzzle (for instance, the patient's family didn't believe the doctor either). Don't worry about them.

Did he think someone wanted to kill/harm him? Was he actually correct? Or was someone manipulating things so he would think that?

Did he think someone was spying on him, had a tap on his phone or was following him around?

Was he hearing some noises he thought he was imagining, that were actually real? Or seeing something? Smelling?

A man and his family approached the good doctor (an expert in schizophrenia) fearing that the man may be developing some dreadful mental illness. Until recently, he had had no mental problems, but lately he had been concerned that the Federal Bureau of Investigation (FBI) had been following him, tracking his movements, and listening in on his phone calls. He was becoming increasingly paranoid. He told the doctor, "I know it's crazy, that it can't be true, but I believe it all the same. You've got to help me." The doctor conducted a full examination but the man was suspiciously lacking in symptoms except for his "delusional" beliefs. The doctor was puzzled and stated that he would need to review the information before rendering a diagnosis. Once the man left, another man arrived and identified himself as a special agent of the FBI. He told the doctor that the FBI had indeed been keeping tabs on the man as part of an investigation.

Later, the man returned to the office expecting a diagnosis, but of course, all the doctor could say was that the man lacked any psychopathology- that there was nothing wrong with him. At the time, he was unable to explain his reasoning, so no one believed him. Once the investigation had been completed, the agent contacted the man and his family to relay the information he had given the doctor.

This is a true story, according to the doctor, embellished somewhat on my part to fill in details I hadn't been given. Thanks for your participation!
