Chester is driving home from work, having recently returned to his job. Suddenly he groans and shouts, "Now everyone knows!"
Why?

"Having recently returned" - he had previously left the job, retired, had time off for health, holiday, family or personal reasons, had a lunch break?
Relevant that he was driving as opposed to walking etc? Was driving all or part of his job? Did he have passengers?
Has he only just realised, as he's driving home, that everyone knows the thing? Did something he saw on the journey make hime realise? Does he have a secret? Is it something he's ashamed of? Is any part of this puzzle to do with secretions?

Did he hear something relevant? Was he listening to the radio? Does everyone know something about him? Is a surprise relevant? A secret? Something embarrassing?

Should we explore his health issues further? Is he now fully recovered? Does he drive the car because of his health? Did people know he was off for health reasons? Did they know what his health reasons were? Is the secret to do with his health? Good or bad secret?

Is he at risk of something now that his secret's out? Getting into trouble? With his employer? Does he see something on his drive aimed directly at him? Or something tenuous which reminds him of something? Is his secret the actual problem with his health itself? Or something related to it?

Is it a roadsign? A billboard? Something public that would claim that Chester's "illness" is fake/not serious?

Does his belief that everyone knows assume that others have seen the sign?

I think I have found the sign, but not sure, as it doesn't seem to make the health condition relevant... Was the sign in Virginia? in an interstate?

Chester had been absent from work due to a bout of Lyme disease, which he had kept secret from his co-workers. But driving home, he passed an exit sign that read:

Chester
Hadlyme

(This is a real exit sign - unfortunately I do not have a picture - on Route 9 in Connecticut.)
